UF&Shands Jacksonville primary care centers offer services for patients during every stage of life, from pediatrics to geriatrics. University of Florida College of Medicine-Jacksonville physicians work with physician assistants and advanced registered nurse practitioners in primary care centers in Duval, Clay, Nassau and Baker counties.

Patient care is our top priority, so our offices offer appointments within 24 hours. Services in many of our centers include annual checkups, well-baby visits, sports physicals and disease prevention, as well as management of chronic medical conditions such as high blood pressure, diabetes and depression.

Because we are affiliated with the UF Health Science Center Jacksonville, patients have access to all of the benefits of an academic health center, including advanced medical treatment from UF specialists such as cardiology, neurology, rheumatology and orthopedics, and the latest in cutting-edge research.

NCQA Patient-Centered Medical Home Recognized Practice

In 2011, the National Committee for Quality Assurance (NCQA) designated the UF College of Medicine–Jacksonville and Shands Jacksonville as Patient-Centered Medical Home providers. UF and Shands Jacksonville are the first health organizations in Northeast Florida as well as the first academic medical group in Florida to receive this designation.

NCQA describes the Patient-Centered Medical Home as a model for care provided by physician practices that seek to strengthen the physician-patient relationship.

With more than 5,000 faculty and staff, our academic health center in Jacksonville is by far the largest UF campus outside of Gainesville. At 37 clinical sites throughout Northeast Florida, UF physicians tallied more than 600,000 outpatient visits and more than 34,000 inpatient admissions in 2010.
